I registered for transportation in the previous school year with Happy Hands and Feet Kidz Transportation, Inc. 

This is my first time registering with Happy Hands and Feet Kidz Transportation, Inc. 

I agree to inform my school secretary or the transportation department as my transportation needs change. I also agree I must be registered with the transportation department and show my pass prior to boarding. By signing below, I certify that all information entered is accurate and correct. *

Weekly Fees Payments must be paid on Friday, prior to service being rendered on Monday Service will NOT be rendered on Monday without payment.*

Fees are not PRORATED; payment is due regardless of if child attends. *

Fees for Spring break and Thanksgiving are your regular weekly rate, due before holidays to maintain your child’s space.*

Fees for Christmas Break are one full week paid to us to maintain your child’s space *

The Child or Children should be ready to board the Van upon arrival, Failure to have student ready upon Van arrival will result in your child missing the Van. We do offer a Courtesy phone call upon arrival but WE DO NOT WAIT for more than 5 minutes. *

Scheduled time of pick up and drop off can change due to enrollments and withdrawals. Notice of time change will be given verbally in advance.*

Happy Hands & Feet Kidz Transportation, reserves the right to cancel service due to conflicts, lack of area participation, or Child Behavior Problem.*

I have received a copy of the policy and Procedures and understand my obligations. If at any time I am unable to fulfill my obligation, I will notify Happy Hands & Feet Kidz Transportation in writing. *
